Summary:Objective:To assess the differences in brain functional connectivity among transcutaneous nerve stimulation (TS), electroacupuncture (EA), manual acupuncture (MA) and sham acupuncture (SA) in ischemic stroke (IS) patients with upper limb dysfunction using resting-state functional magnetic resonance imaging (rsfMRI).Methods:148 IS patients with upper limb dysfunction were randomly assigned to TS group (<italic>n</italic>=37), EA group (<italic>n</italic>=36), MA group (<italic>n</italic>=38) and SA group (<italic>n</italic>=37). All participants received upper limb motor function evaluation and activity of daily life evaluation before and after four-week period of TS, MA, EA and SA at Quchi and Waiguan. Ten patients with the left cerebral infarction from each of four groups were chosen to received rsfMRI scanning and their ipsilateral primary motor cortex was chosen as seed.Results:Compared with the SA group, the Fugl-Meyer upper limb function scale (FMU) and modified Barthel index (MBI) scores in EA group, MA group, TS group were increased respectively after four weeks treatment (<italic>P</italic>&lt;0.05). Compared with the EA group and MA group, the FMU score in TS group was increased (<italic>P</italic>&lt;0.05). Compared with the MA group, the FMU score in EA group was increased (<italic>P</italic>&lt;0.05). After the treatment, the functional connectivity of left M1 with the left precentral gyrus, left postcentral gyrus, the right cerebellum, and right supplementary motor area in TS group were increased; the functional connectivity of left M1 with the left postcentral gyrus, middle frontal gyrus, inferior parietal lobule and supplementary motor area in EA group were increased after treatment;the functional connectivity of left M1 with the left precentral gyrus and postcentral gyrus in MA group were increased, with the left cerebellum was decreased;the functional connectivity of left M1 with the left postcentral gyrus in SA group were increased, but the connection strength was smallest within four groups, and with the left middle frontal gyrus and precentral gyrus was decreased.Conclusion:TS, EA and MA at Quchi and Waiguan can improve upper limb function of IS patients with upper limb dysfunction.rsfMRI study reveals the effect may related to the increase of functional connectivity of left M1 with the precentral gyrus, postcentral gyrus, middle frontal gyrus and supplementary motor area.
