Summary:Urethral diverticulum is a congenital or an acquired abnormal outpouching of the urethra. This condition can be seen in all age groups and across sex divides. The causes can be congenital or acquired. In adults, the acquired causes abound and commonly include post-trauma, post-infective urethritis and sequelae of hypospadias surgery. This is a report of a 32-year-old trader presenting with a three-week history of distal phallic swelling associated with lower urinary tract symptoms who had missed diagnosis of tricorporal penile fracture two weeks before the onset of symptoms at a secondary health facility. The clinical and imaging investigations confirmed distal penile urethral diverticulum for which he had urethroplasty. The urethroplasty failed due to infective complications warranting redo-urethroplasty later. Open surgical exploration of the phallus and urethroplasty is vital in patients with urethral diverticulum but may be compromised by infective complications.
