    The predictive value of mean platelet volume to platelet count ratio combined with APACHEⅡs for short-term prognosis of patients with acute kidney injury treated by continuous renal replacement therapy by TANG Ying-ying, NI Xun, Lv Jian-nong

    “…Objective To investigate the predictive value of mean platelet volume(MPV)to platelet count(PLT)ratio(MPR)combined with acute physiology and chronic health evaluation Ⅱ(APACHE Ⅱ)scoring for the short-term prognosis of patients with acute renal injury(AKI)treated with continuous renal replacement therapy(CRRT).Methods A total of 138 AKI patients who received CRRT in our hospital from January 2016 to October 2019 were retrospectively included.According to their survival conditions at discharge,they were divided into survival group(n=80)and death group(n=58).The value of MPR,APACHE Ⅱ score and their combination for predicting the death of AKI patients was analyzed by ROC curve.The independent risk factors for poor prognosis were evaluated by multivariate logistic regression analysis.Results(1)MPV,MPR and APACHE Ⅱ scores of patients in the death group were significantly higher than those in the survival group[(11.9±1.1)fl vs.(11.0±1.1)fl,P<0.01;0.11(0.09,0.14)vs.0.08(0.06,0.11),P<0.01;(25.0±3.5)score vs.(19.5±3.4)score,P<0.01].PLT count of patients in death group was significantly lower than that of survival group[(108.9±28.0)×10~9/L vs.(146.8±41.4)×10~9/L,P<0.01].(2)The cutoff values of MPR,APACHE Ⅱ score and the combination to predict the death of AKI patients were 0.08,24 points and 0.54,respectively.The AUC of death in AKI patients predicted by MPR combined with APACHE Ⅱ was significantly higher than that of MPR and APACHE Ⅱ alone[0.892(0.828-0.938)vs.0.737(0.656-0.809),P<0.01;0.892(0.828-0.938)vs.0.838(0.766-0.896),P<0.05].(3)APACHE Ⅱ score(≥24),MPR combined with APACHE Ⅱ score(APACHE Ⅱ score≥24,and MPR≥0.08),diuretics administration were the independent risk factors for poor prognosis in AKI patients[HR(95%CI):2.311(1.034-5.166),3.533(1.541-8.098),34.816(7.475-162.149),all P< 0.05].Conclusions MPR combined with APACHE Ⅱ score can well predict the short-term prognosis of AKI patients treated with CRRT.…”

    “…ObjectiveTo explore the relationship between intact parathyroid hormone (iPTH),fetal globulin A (FA),serum creatinine/hemoglobin ratio (Scr/Hb) and calcium and phosphorus metabolism disorder in chronic renal failure patients with chronic kidney disease (CKD) on maintenance hemodialysis (MHD) and examine the predictive value of each parameter for carotid atherosclerosis.MethodsFrom July 2014 to July 2020,113 CKD patients on MHD were recruited and divided into two groups of sclerosis (<italic>n</italic>=30) and non-sclerosis (<italic>n</italic>=83).The baseline levels of iPTH,FA and Scr/Hb of two groups were recorded.The related influencing factors of carotid atherosclerosis,parameters of calcium and phosphorus metabolism and their correlations were examined.ResultsBaseline phosphorus,iPTH and Scr/Hb in sclerosis group were higher than those in non-sclerosis group [(1.89±0.42)mmol/L、305.26(152.63,608.19)μg/L、7.16±2.19 <italic>vs </italic>(1.30±0.36)mmol/L、238.47(118.05,506.11)μg/L、4.38±1.38,<italic>P</italic>&lt;0.001],FA was lower than that of non-sclerosis group [(74.62±20.61)mg/L <italic>vs </italic>(95.33±25.80)mg/L,<italic>P</italic>&lt;0.05]; mean values of iPTH,FA and Scr/Hb were selected as boundary,&lt;mean value was low level,≥ mean value high level.High levels of iPTH and Scr/Hb were higher than those with low levels [(1.60±0.39)mmol/L <italic>vs </italic>(1.18±0.33)mmol/L,(1.71±0.55)mmol/L <italic>vs </italic>(0.97±0.31)mmol/L] and those with high levels of FA lower than those with low levels [(1.25±0.34)mmol/L <italic>vs </italic>(1.72±0.40)mmol/L,<italic>P</italic>&lt;0.05]; iPTH,Scr/Hb and phosphorus were positively correlated while FA was negatively correlated with phosphorus (<italic>P</italic>&lt;0.05);iPTH,FA,Scr/Hb were still associated with IMT (<italic>P</italic>&lt;0.05); the area under the curve (AUC) of iPTH,FA and Scr/Hb for predicting carotid atherosclerosis were 0.742,0.776 and 0.790 respectively.It was the largest at 0.918 for combined predicting.The optimal predictive sensitivity and specificity were 93.33% and 85.54% respectively.ConclusioniPTH,FA and Scr/Hb are closely correlated with disorder of calcium and phosphorus metabolism in chronic renal failure patients with CKD on MHD.These parameters offer high application values in predicting carotid atherosclerosis.…”
