    “…Thyrotoxic encephalopathy is a rare clinical entity associated with the two autoimmune thyroid disorders1: Hashimoto’s thyroiditis (more commonly) or Grave’s disease.2,3 It is more prevalent in women and presents with altered sensorium, behavioural or cognitive disturbances and rarely with psychosis or parkinsonism.4–6 Mechanisms remain poorly understood, although it is suggested that encephalopathy associated with autoimmune thyroid disease is thought to involve cerebral vasculitis with endothelial inflammation or immune complex deposition leading to global or focal hypoperfusion in the brain.7 It is a diagnosis of exclusion.8 It is characterised by high levels of free tri-iodothyronine (fT3) and free tetra-iodothyronine/thyroxine (fT4), with suppressed thyroid-stimulating hormone (TSH) as well as high titres of thyroid autoantibodies, such as thyroid-stimulating hormone receptor antibodies (TRAb) and anti-thyroid peroxidase antibodies (TPOAb).9 There is significant improvement of symptoms following administration of corticosteroids.10 Here, we describe a case of thyrotoxic encephalopathy caused by seronegative thyroiditis. …”
