    Analysis of related factors in maintenance hemodialysis patients with renal anemia by NI Hua, PENG Jia-qing

    “…Objective To observe the renal anemia in maintenance hemodialysis(MHD)patients,analyze the related factors,and understand the relationship between hepcidin expression level and renal anemia in MHD patients.Methods This cross-sectional study was carried out in 128 MHD patients,including72 males and 56 females,with a mean age of(53.50±13.26)years and a mean dialysis vintage of(32.13±17.04)months from August 1 to 28,2016.The clinical data of the patients were collected,and parameters such as routine blood test,C-reactive protein(CRP),Hepcidin,serum iron,ferritin,unsaturated iron-binding capacity(UIBC),electrolyte,folic acid,vitamin B12,parathyroid gland hormone(iPTH)and electrolyte were measured before the mid-week hemodialysis.Blood creatinine and urea nitrogen were detected before and after dialysis,and single chamber urea clearance index(sp Kt/V)was calculated.Results The mean hemoglobin in 128 patients was(98.54±16.93)g/L.Among them,hemoglobin ranged in 110-120 g/L in 28 cases,accounting for 22%.Pearson correlation analysis showed that hemoglobin was significantly positively correlated with leukocyte(r=0.279,P=0.001),erythrocrit(r=0.279,P=0.002),serum iron(r=0.188,P=0.033)and dialysis adequacy(r=0.215,P=0.015),but negative with hepcidin(r=-0.302,P=0.001),serum ferritin(r=-0.207,P=0.019),UIBC(r=-0.195,P=0.027),iPTH(r=-0.264,P=0.003)and C reaction protein(r=-0.203,P=0.022).Multiple linear stepwise regression model results suggested that the hemoglobin protein was positively correlated with erythrocrit(P<0.01),but negatively with hepcidin(P=0.001)and iPTH(P<0.05).Conclusions Anemia in MHD patients is closely related with number of white blood cells,hematocrit,hepcidin,serum iron,dialysis adequacy,UIBC,iPTH,C reaction protein,serum iron and other factors,Little correlation with folic acid and vitamin B12.…”
