    “…Objective To analyze the distribution of left ventricular hypertrophy(LVH) and its effect on renal progression in patients with chronic kidney disease(CKD) of stages 3<sup>4</sup>.Methods We collected the baseline data of 61 patients with CKD stages of 3<sup>4</sup> who were followed up in Nephrology outpatient of Hubei Provincial Hospital of TCM from February 2012 to February 2016,and found out the distribution of LVH.By following up regularly,we recorded the outcome events including serum creatinine(SCr) double,estimated glomerular filtration rate(eGFR)<15 ml·min<sup>-1</sup>·(1.73m<sup>2</sup>)<sup>-1</sup>or 50%reduction,renal replacement therapy or death.And we divided 61 cases into renal function progression group and non-progression group,compared the data between two groups,and used COX risk regression model to analyze risk factors on prognosis of kidney.Results The average follow-up time was(34.70±9.54) months.One case dropped out,and 22 cases entered outcome events,including 12 cases with SCr doubles/50%reduction of eGFR or <15 ml·min<sup>-1</sup>·(1.73 m<sup>2</sup>)<sup>-1</sup>,8 cases undergoing maintain hemodialysis and two deaths.The prevelence of left ventricular dilatation,LVH and left ventricular diastolic dysfunction in CKD stages 3<sup>4</sup> was 22.95%,37.70%,and60.66%respectively.The differences in systolic blood pressure,SCr,eGFR,interventricular septum thickness(IVST),left ventricular end-diastolic diameter(LVEDd),left ventricular mass(LVM),left ventricular mass index(LVMI) and rate of left ventricular enlargement and LVH were significant between renal function progression group and non-progression group(P<0.05).COX single factor regression analysis showed that systolic blood pressure(P = 0.013),LVM(P = 0.003),LVMI(P =0.001),left ventricular enlargement(P<0.001) and LVH(P = 0.009) could endanger the patients of CKD of stages 3<sup>4</sup> into the end point events.Multivariate COX proportional hazards regression analysis(enter method) showed risk factors associated with renal function progression in patients of CKD of stages 3<sup>4</sup> were systolic blood pressure(b=1.075,HR=2.931,P = 0.025),LVH(b=1.319,HR= 3.740,P = 0.048).Conclusions LVH was common observed in patients with CKD of stages 3<sup>4</sup>.Early paying attentions to cardiac ultrasound,and timely intervening and controlling blood pressure and structural abnormality of left ventricle are necessary.…”
