Analysis of correlated factors on maintenance hemodialysis patients with parathyroid dysfunction
Published 2017-01-01“…Objective To analyze the related factors on the maintenance hemodialysis patients with parathyroid dysfunction,and further analyzed the independent risk factors of abnormal parathyroid hormone in MHD patients.Methods 85 patients were divided into PTH reduced group(iPTH<150 ng/L),PTH normal group(150 ng/L≤iPTH≤300 ng/L) and PTH increased group(iPTH>300ng/L) according to the levels of intact parathyroid hormone.Hamilton depression scale(HAMD 24 items) was adopted to evaluate depression status of patients.The indicators of age,dialysis age,morbidity of diabetes,prevalence of depression,body mass index(BMI) and serum biochemical variables were compared in this study.Multiple linear regression was used to analyze the correlation of LPTH and the above indicators.Moreover,multifactor logistic regression was also employed to analyze the risk factors on iPTH levels abnormity.Results Compared with PTH normal group,serum calcium levels were lower(P<0.05) and the diabetes morbidity rate and depression morbidity rate were higher in the PTH reduced group(P<0.05),the hemoglobin was lower,age and serum creatinine level were higher in the PTH increased group.The serum iron and transferrin saturation of PTH reduced group and PTH increased group were lower in comparison with PTH normal group.Dialysis age,C-reactive protein,hematocrit,ferritin total iron binding force and serum phosphorus of the three groups showed no statistical significance(PX).05).The result of multiple logistic linear regression showed that the iPTH was positively correlated with BMI and age(r= 0.040,0.051),and negatively correlated with hemoglobin and diabetes(r=-0.039,-0.357).The logistic linear regression showed that diabetes was the independent risk factors for reduced iPTH,and BMI,serum creatinine were the independent risk factors for increased iPTH.Conclusions The iPTH level in MHD patients was positively correlated with BMI and age,and negatively correlated with hemoglobin and diabetes.Diabetes,BMI and serum creatinine were independent risk factors of the abnormal level of PTH.…”

Analysis on influencing factors of hemodialysis adequacy in maintenance hemodialysis patients
Published 2020-01-01“…Objective To explore the influencing factors of adequacy of single hemodialysis in maintenance hemodialysis(MHD) patients.Methods A total of 109 patients who underwent MHD in Blood Purification Centre of Aviation General Hospital in March 2019 were enrolled in this study.The data on pre-HD and post-HD gender,age,body mass index(BMI),primary disease,vascular access and blood urea nitrogen(BUN),hemodialysis time,ultrafiltration volume(UF),blood flow rate(BFR),dry body weight and ratio of UF in single hemodialysis were recorded.According to spKt/v results,the patients were divided into two groups:qualification group(spKt/V≥1.2) and disqualification group(spKt/v<1.2).The influencing factors were analyzed.Results Univariate analysis showed that spKt/V was correlated with gender,dialysis associated coagulation,BFR,arteriovenous fistula(AVF)and dry body weight(P<0.05).Binary Logistic regression showed that gender(OR=4.345,95%CI 1.240~15.070,P<0.05),dialysis associated coagulation(OR=5.497,95%CI 1.213~27.125,P<0.05),BFR(OR=0.984,95%CI 0.969~0.998,P<0.05) and AVF as vascular access(OR=0.105,95%CI 0.012~0.889,P<0.05) were independent influencing factors of spKt/V for single hemodialysis.Conclusions The single hemodialysis adequacy in MHD patients is closely associated to gender,dialysis associated coagulation,BFR and AVF.…”

Clinicopathological characteristics and related factors of IgA nephropathy with hyperuricemia
Published 2021-01-01“…Objective To explore the clinicopathological characteristics of IgA nephropathy with hyperuricemia and examine the related influencing factors.Methods A total of 320 patients with primary IgA nephropathy receiving an initial renal biopsy from January 1,2015 to December 31,2019 were selected as research subjects.According to the serum level of uric acid, they were divided into two groups of hyperuricemia(n=111)and normal uric acid(n=209).The inter-group differences of clinicopathological parameters were compared.The relevant factors of IgA nephropathy with hyperuricemia were analyzed by a regression model.Results Among 320 patients with IgA nephropathy, 34.7% had hyperuricemia.Proportion of male, systolic blood pressure, diastolic blood pressure, uric acid, hemoglobin, triglyceride, urea nitrogen, creatinine and 24-hour urine protein were higher in hyperuricemia group than those in normal uric acid group(P<0.05).However, high density lipoprotein and estimated glomerular filtration rate(eGFR)were lower than those in normal uric acid group(P<0.05).In terms of pathological changes, proportion of segmental glomerulosclerosis/adhesion and tubular atrophy/interstitial fibrosis>25% was higher in hyperuricemia group than that in normal uric acid group(P<0.05);further pairwise inter-group comparison indicated that proportion of tubular atrophy/interstitial fibrosis>50% group was higher than that in tubular atrophy/interstitial fibrosis<25% and 26%~50% groups(P<0.05).And eGFR was an independent factor of IgA nephropathy with hyperuricemia(P<0.05).Conclusion Clinicopathological changes of IgA nephropathy patients with hyperuricemia are more serious than those with normal serum uric acid.It implies that hyperuricemia may be correlated with the progression and poor prognosis of IgA nephropathy.A comprehensive control of uric acid, blood pressure and blood lipid helps to improve the prognosis of patients.…”
