Clinical value of mean platelet volume/lymphocyte ratio and degree of coronary artery disease in patients with chronic kidney disease
Published 2022-05-01“…ObjectiveTo investigate the relationship between mean platelet volume to lymphocyte ratio (MPVLR) and the degree of coronary artery disease in patients with chronic kidney disease (CKD) and its clinical significance.MethodsThe clinical data of 111 patients with chronic kidney disease diagnosed by coronary angiography in our affiliated hospital in recent 6 years were analyzed retrospectively.The Gensini scoring was used to assess the severity of coronary artery disease.The data were divided into three groups: group A (mild,29 cases) had 0-30 points;Group B (moderate,23 cases) had 31-60 points,and group C (severe,59 cases) had >60 points.First,the related factors affecting coronary artery disease were analyzed by univariate analysis.On this basis,the risk factors and protective factors were determined by Logistic regression analysis.Finally,the predictive value was determined by ROC curve.ResultsUnivariate analysis showed that there was no significant difference in general data among the 3 groups (<italic>P</italic>>0.05),but there were significant differences in laboratory indicators such as albumin,uric acid,high density lipoprotein cholesterol (HDL-C) and MPVLR (<italic>P</italic><0.05).High MPVLR level was an independent risk factor for high Gensini integration (<italic>OR</italic>=1.115,95%<italic>CI</italic> 1.009-1.239,<italic>P</italic>=0.047),and HDL-C was a protective factor.ConclusionsIn patients with chronic kidney disease,high MPVLR is an independent risk factor for the severity of coronary artery disease,and has the application value in predicting the severity of coronary artery disease.…”

Association of baseline neutrophil-to-lymphocyte ratio with primary peritonitis in peritoneal dialysis patients
Published 2021-01-01“…Objective To retrospectively analyze whether a high neutrophil-to-lymphocyte ratio(NLR)was associated with the risk of initial peritonitis in peritoneal dialysis(PD)patients.Methods A total of 119 patients with end-stage renal disease underwent PD catheterization from August 2013 to July 2018 and regular follow–ups were conducted.They were divided into the groups of high NLR(NLR≥2.65)and low NLR(NLR<2.65)according to the median baseline level of NLR.The clinical data of two groups were compared.Logistic and linear regression models were employed for analyzing the correlation between baseline serum NLR and the occurrence of initial peritonitis.The diagnostic value of NLR to PD-associated peritonitis(PDAD)was evaluated by ROC curve.Results A total of 119 patients were recruited.The incidence of peritonitis was higher in high NLR group.Univariate logistic regression revealed a higher risk of peritonitis at baseline with a high level of NLR(OR=2.555,95%CI 1.684~3.878,P<0.01)had a higher risk of peritonitis.Multivariate analysis indicated that a high baseline level of NLR(OR=2.467,95%CI 1.439~4.232,P<0.01)was an independent risk factor for peritonitis.Univariate linear regression analysis showed that a high baseline level of NLR(standardized β=-0.352,P<0.01)was negatively correlated with dialytic age;multivariate linear regression analysis indicated that a high baseline level of NLR(standardized β=-0.340,P<0.01)was an independent risk factor for dialytic age.The area under the ROC curve was 0.813 when peritonitis predicted by baseline NLR peritonitis(P<0.01)and the maximal index was 0.595(sensitivity and specificity 71.4% and 88.1% respectively).The corresponding optimal cut-off point was 3.64.Conclusions Higher serum baseline level of NLR is associated with an elevated risk of PDAP.…”

Association of secondary hyperparathyroidism with mild cognitive impairment in end-stage renal disease patients
Published 2021-01-01“…Objective To explore the relationship between secondary hyperparathyroidism(SHPT) and mild cognitive impairment(MCI)in end-stage renal disease(ESRD)patients.Methods A total of 287 renal replacement therapy patients were enrolled.Based upon the diagnostic criteria of SHPT,MCI patients were divided into SHPT and NSHPT groups.The relationship between intact parathyroid hormone(iPTH)level and MCI incidence in SHPT patients was analyzed by Spearman’s correlation analysis.Multivariate logistic regression was employed for examining the influencing factors of MCI.Occurrence of MCI or not and iPTH level were used as variables for plotting the receiver operating characteristic curve(ROC)and assessing the value of iPTH level for predicting MCI in ESRD patients.Results Among 189 MCI patients,the incidence rate was 65.9%(SHPT 82%).Spearman’s correlation analysis revealed that the incidence of MCI rose with a higher level of iPTH(r=0.598,P<0.01).Multivariate Logistic regression analysis showed SHPT,iPTH,25-hydroxyvitamin D\[25(OH)D\],alkaline phosphatase(ALP),dialytic mode and urea clearance index(Kt/V)were influencing factors of MCI(P<0.05).The results of ROC curve showed that the area under the curve of iPTH level predicting MCI in ESRD was 0.838 and the cutoff value was 492.25 ng/L with a sensitivity of 75.7% and a specificity of 95.9%.Conclusions SHPT is an influencing factor for MCI in ESRD patients.The higher the level of iPTH,the greater the incidence of MCI.Patients with iPTH levels>492.25 ng/L have an elevated risk of MCI.…”

Relationship between hemoglobin variability with hospitalizations/mortality among maintenance hemodialysis patients
Published 2015-01-01“…Objective To investigate the correlation between hemoglobin variability with hospitalizations/mortality of maintenance hemodialysis(MHD) patients.Methods In a retrospective study,we analyzed the hemoglobin variability and hospitalization and mortality of MHD patients from June,2012 to June,2014.Data of normal distribution were performed by analysis of variance.The X~2test with Yates correction and Fisher’s exact test were used for no-numerical date.Correlations between hemoglobin variability and age,dialysis age and mean hemoglobin were analyzed with Spearman’s correlation test.To predict the hospitalizations/mortality,logistic regression analysis was performed.Results In a total of 105 MHD patients,mean hemoglobin was(108.39±8.81)g/L,mean hemoglobin SD was(11.43±4.62)g/L.95.24%of patients had a high fluctation of hemoglobin.All the patients were divided into outpatients group(42 patients),in the hospital group(45 patients)and dead group(18 patients).The age of three groups were(64.02±13.02)years,(69.44± 13.47)years and(81.44±10.15)years respectively.The dialysis age of three groups were(9.23±6.42)years,(6.80±4.47)years and(4.98±3.00)years.The mean hemoglobin were(110.46±7.12)g/L,(108.51±8.12)g/L and(103.29±12.00)g/L respectively.Hemoglobin SD were(9.96±3.74)g/L,(11.98 ± 5.29)and(13.47 ± 3.73)g/L.The differences between three groups had statistically significance.Hemoglobin variability was significantly correlatied with the erythropoietin adjustments(r= 0.209,P = 0.032).Logistic regression analysis revealed that the dialysis age and hemoglobin SD were the influencing factors of hospitalizations/mortality.Conclusions Hemoglobin variability is common in MHD patients.High hemoglobin fluctuation is the risk factor predicting hospitalizations and mortality.…”
