description Our objective is to report and discuss a rare (under-recognized) case of visual release hallucinations also known as Charles Bonnet syndrome (CBS). Usually, it spontaneously resolves within a few years in most cases. Most patients who experience hallucinations as part of CBS are not bothered by the symptoms and, hence, are not candidates for pharmacological treatment. Recognizing a patient’s report of visual hallucinations as a symptom of vision impairment can avoid misdiagnosis, unnecessary investigation, and possible mismanagement. The patient was treated with tablet risperidone and was reassured that his visual hallucinations might be due to his eye disease and thus was not real. Although there was no improvement in hallucinations, he did not report the hallucinations to be as distressing as before and gained the capacity to live with his continuing visual hallucinations with no need for medications. This case report highlights the importance of counseling in CBS about the nature of the hallucinations.
